Child safeguarding incident notification system

Please note that even in situations where there is no named or identified child, if there is known or suspected abuse or neglect and the child or children have died or have been seriously harmed, local authorities can still submit a notification using the limited initial information available to them.

Use this service to:

  • notify the Child Safeguarding Practice Review Panel if it’s known or suspected that a child has been abused or neglected and;
    1. the child dies or is seriously harmed in the local authority’s area.
    2. the child dies or is seriously harmed outside England while normally resident in the local authority’s area.
  • notify the Secretary of State for Education, and Ofsted of the death of a looked after child regardless of known or suspected abuse or neglect
  • notify the Secretary of State for Education and Ofsted of the death of a care leaver up to and including the age of 24

Before you start

You should refer to the guidance to report the death or serious harm of a child or care leaver for information on what you need to report, information needed to complete a report and information on submitting your incident report.

Make sure you have prepared all the data you need before you start filling out the reporting form. The child safeguarding incident notification system times out after 20 minutes of inactivity. For security purposes, information entered into the form will not be saved after this period of inactivity.
